How to Apply

We welcome your interest in employment with South Bend Medical Foundation. It is our policy to only accept applications for positions that are currently open.

To apply, please do the following:

  • Click on selected position
  • Click on link as indicated in the job information section
  • Both the employment application as well as the online sections following the job information must be completed

Note:

  • The employment application is computer ready - allowing you to type your information
  • The employment application should be submitted via the Attach a Resume section of the online application.

If you are having trouble attaching the formal application, you may also complete the application process by downloading the application below and completing it by hand. These applications can be faxed, mailed or emailed.

All applications submitted by qualified candidates will be reviewed in Human Resources and given every consideration. Applications will remain active for three months from the date submitted and will only be considered for positions that are currently open.

For consideration, all interested candidates must complete a employment application. You may also apply in person between 8:00 a.m. - 5:00 p.m. Monday through Friday using our Main Street entrance. If a position becomes available for which you would like additional information, please contact the Human Resources Department.
