    Disinfect venipuncture site with providone-iodine prep pad from collection kit
    Use needle and needle/tube holder from kit to perform venipuncture
    Fill both gray top tubes from kit per approved venipuncture technique
    After completion of venipuncture, mix both tubes well, inverting end-to-end at least 8–10 times
    Seal each tube using pink “A” and light blue “B” specimen seals from bottom of "Custody and Control Form"
        Note: Do not use red security seals from kit (to be discarded)
    Ensure that pink and blue seals are applied in a linear fashion down sides of each tube; do not wrap seals around either tube
    Complete date information on both tube seals and have donor initial each seal in appropriate area
    Position sealed blood samples back into tube case from kit and snap shut
    Place sample tube case within “Biohazard” bag and slip “Custody and Control Form” into outside pocket on bag
    Enclose bag containing samples and form within original kit box and secure by placing “BOX SEAL” sticker over opening edge of box
