Used in monitoring therapeutic vancomycin level in blood
  • Clinical Significance:
Vancomycin is a complex glycopeptides antibiotic, which has been used to treat penicillinase-producing staphylococci. It is the drug of choice for the treatment of methiciliin and related beta lactam antibiotic resistant Staphylococcus aureus as well as for the treatment of serious gram-positive infections where allergies to pencillin or cephalosporin play a role. Vancomycin is also used in the treatment of antibiotic-induced enterocolitis associated with Clostridium difficile and streptococcal or enterococcal endocarditis, the latter in conjunction with an aminoglocyside, when penicillin or ampicillin is not an option.
  • Reference Range:
Vancomycin Trough Level: Minimum serum concentration for infection control is 10 µg/mL. A target therapeutic range of 15-20 µg/mL for a trough level is recommended for complicated infections. There are limited data suggesting a direct causal relationship between toxicity and serum concentrations.
  • Critical High Value:
    ≥ 80.1 µg/mL
  • Additional Test Info:
Vancomycin Peak Level: No longer routinely recommended for therapeutic monitoring.
