Peak levels are the best guide to therapy
– For rapid-release preparations, collect peak level 2 hours after dose
– For slow-release preparations, collect peak 3-7 hours after dose
Trough level – Collected immediately before next dose
Document time and type of medication on requisition - Spec Process:
If specimen will not be sent to SBMF on day collected: - Clot 30 minutes - Promptly centrifuge 15 minutes - Immediately transfer serum to separate plastic tube and refrigerate - Spec Store Transport:
Refrigerated - Spec Stability:
1 week refrigerated (2-8°C)
3 months frozen (-20°C) – Do not use frost-free units that undergo repeated freeze/thaw cycles - Methodology:
Particle Enhanced Turbidimetric Inhibition Immunoassay (PETINIA) - Use:
Used in monitoring levels of theophylline to ensure proper therapy and in diagnosis and treatment of theophylline overdose - Clinical Significance:
Theophylline is a methylxanthine derivative that is widely used in the treatment of asthma, obstructive lung disease and neonatal apnea. The primary therapeutic effect of theophylline is due to relaxation of bronchial smooth muscle; however theophylline has a variety of other effects. These include stimulation of the central nervous system and medullary respiratory centers, decreased peripheral vascular resistance, cardiac stimulation, and diuresis. - Reference Range:
0-4 weeks: 5.0-10.0 µg/mL
Greater than 4 weeks: 10.0-20.0 µg/mL - Critical High Value:
Birth to 4 weeks: >15.0 µg/mL
Greater than 4 weeks: >25.0 µg/mL - Day Run:
