  • Clinical Significance:
    The porphyrias are a group of genetic abnormalities of heme synthesis in which abnormal concentrations of heme precursors accumulate. Many conditions that interfere with iron metabolism in the developing erythrocyte will induce an increase in ZPP and porphyrins. Such conditions include chronic inflammatory processes, iron deficiency, and lead exposure. Depending upon clinical circumstances, other tests, such as whole blood lead or serum ferritin, may be necessary for evaluation. Serum porphyrins are more specific for inherited porphyrin disorders, such as erythropoietic protoporphyria. Zinc protoporphyrin (ZPP) accumulates in erythrocytes as a result of chronic lead absorption or iron deficiency anemia.

    ZPP OSHA Occupational threshold: <100 µg ZP/dL blood.
    It is recommended that a hematocrit be determined whenever a confirmed ZPP of 50 µg ZP/dL whole blood is obtained to rule out a significant underlying anemia.
    OSHA requires ZPP whole blood testing in units of µg/dL.
