Determine if a positive DAT is due to IgG or complement coating the patient's red blood cells - Clinical Significance:
A positive DAT with a negative IgG test implies that the positive DAT is due to complement. Red cells may be coated with both IgG and complement. Positive tests for complement are found in: warm autoimmune hemolytic anemia (complement alone or with IgG); cold agglutinin disease and paroxysmal cold hemoglobinuria; drug therapy, drugs causing Coombs positivity (e.g., quinidine, insulin, antihistamines, sulfonamides, phenacetin, and quinine). Positive tests for IgG are found in warm autoimmune hemolytic anemia, hemolytic disease of the newborn, transfusion reactions, and with use of drugs including Aldomet, penicillin, cephalosporin drugs, and mefenamic acid. Elution and antibody identification studies are performed on selected IgG-positive samples to check for any apparent specificity. - Reference Range:
